Identify personal nutritional and health priorities in conjunction with the health care team.
Identify resources for client education re: whole foods, foods as medicine, weight loss, maintenance of a healthy appropriate body weight and composition, optimal food plans for medical conditions, provide support for pantry and kitchen restock, grocery lists, meal prep, and strategies for eating out.
Set SMART goals weekly. Educate, investigate, and motivate client to reach short, midrange, and long term nutrition goals.
Provide accountability to meet SMART goals.
